NOTES:

Assume Today's date is  01 September 2020.

You are the radiographer working in the X-ray Department of St Francis Hospital. The Surgery Registrar  has asked you to explain the reason why his patient Ms Williams's scheduled X - ray delayed.

PATIENT DETAILS:

Name: Ms Dorothy Williams

Date of Birth: 05 March 1973 (46 yo)

Address: 201, Lake street. NY 213

Request from: Dr Jerald Fox (Chief Physician, City Hospital.

Exam required: X-ray (Head of the humerus)

Reason for procedure: Severe damage on the shoulder.  ?fracture head of humerus.

Medical history: Physically challenged, lower rt limb re: polio.

Recent history: sustained severe injury in a car accident.

Radiography Notes:

03.00 am: 

Request for X-ray - Pt: Ms Dorothy Williams. Pt accompanied by husband, Mr Williams.

Advised Mr Williams must

* be covered with lead apron.

* stand behind the screen during exposure/as far the primary beam as possible.

03.20 am: 

The Chief Radiographer (Mr Whytt) notices a specific information missing in the request form. Does not mention left side or rt side.

03.25 am

Contact Dr Jerald Fox (Chief Physician) City Hospital. Doctor with an emergency case.

03. 45am

Informs X-ray of both sides. Radiographer with another patient.

05.10 am

X-ray taken. Report ready by 06.00 am.

Report dispatched by 6.30 am.

Writing Task

Using the information given in the case notes, write a letter to   Dr Edmund explaining the reasons why an urgent X-ray request of Ms Williams was delayed.  Address the letter to Dr Jerald Edmund,   The Surgery Registrar, St Francis  Hospital, NY 231).
