Candidate: I'm sorry to hear about your concerns.
In the meantime, could you please tell me your name for my records, please?

Patient: Well, I'm 26 years.
Candidate: (* Confirm reason for visit) So, you have loss of appetite, but feeling nauseating at the thought of foods, and loss of body weight. Am I correct.
Patient: Exactly. That's what worrying me.
Candidate: Well, relax and we are here to help you,
(Find out about the patient's appetite.). May I know about your appetite?
Patient: That's my real concern. I have no appetite and the thought of food is still nauseating.
Candidate: I understand.
(* Explore foods eaten yesterday (What foods, how well tolerated: any nausea, vomiting, diarrhoea, stomach cramps, etc.). Can you recollect what did you eat yesterday, David?
Patient: Well, I remember well, I had toast in the morning, a cup of chicken noodle soup for lunch, and rice with canned pink salmon for dinner. However, I felt quite uncomfortable, and needed to go to the bathroom straight after each meal. I have vomited everything I ate, and I had diarrhoea and stomach cramps. It's frustrating.
Candidate: I'm sorry to hear that.
(* Give information about Crohn's disease and diet ) Based on the signs and symptoms your complaints, it is evident that you are suffering from a condition called Crohn's disease.
David, how much do you know about Crohn's disease?

Candidate. (* Give information about Crohn's disease and diet (e.g., no prescriptive diet, dependent on individuals, etc.) . Suggest how low-fibre diet (e.g., when disease is active /in flare, limiting or avoiding high-fibre foods: raw green vegetables, whole nuts/grains, fruits with skin and seeds, etc.) Yes, it's a type of inflammatory bowel disease, and there's no prescriptive diet for it, and it is dependent on individual.
Patient: Are there no remedy for this disease?
Candidate: Indeed there are a few. ( Suggest how low-fibre diet (e.g., when disease is active /in flare, limiting or avoiding high-fibre foods: raw green vegetables, whole nuts/grains, fruits with skin and seeds, etc.) What I suggest is low-fibre diet. That means when the disease is active you have to limit or even avoid high -fibre foods such as raw green vegetables, whole nuts and grains, fruits with skin and seeds.
Am I clear to you, David?
Patient: Thank you, yes it's clear.
Candidate: (Recommend keeping food diary (establishing patterns of symptoms/tolerance, identifying issues, etc.).
Moreover, it's a good idea to keep a food diary.
Patient: Excuse me, but what's a food diary?
Candidate: That's a good question. The food diary helps to establish patterns of symptoms and tolerance. and identify the issues.
Is that clear for you?
Patient: Yes, I got it, thank you.
I think it's a good idea to keep a food diary because it helps me to find out which food items I should take or avoid.
However, there are a lot of foods that you say I can't eat. It's frustrating.
Candidate: (* Reassure patient about diet (e.g., lots of suitable/enjoyable foods available, etc.)
Please don't panic because there are a lot of suitable and enjoyable foods available.
Patient: Is it? Could you please suggest some?
Candidate: (Suggest additional ways of ensuring better daily nutrition (e.g., multivitamins, 4-6 small meals daily, adequate hydration, meal preparation in advance, simple cooking technique: poaching, steaming, etc.).
Well, I will suggest some extra ways of ensuring better daily nutrition. You can take multivitamins. Also, eat four to sic small meals daily. drink enough fluids to keep you hydrated, and prepare the meals in advance. Is that OK, David?
Patient: Well that sounds great. Anything else?
Candidate: Indeed. Why don't you try some simple cooking, such as poaching and steaming. How do you find it?
Patient: Well, that's easy and I like it.
Candidate: Find out any other questions.
Do you have any further questions?
