1. Past tense and present perfect
Simple past: a finished event at a definite time
(e.g., on 3 June / yesterday/in 2010)
.... was admitted/ presented/ underwent.
Ms Williams was admitted on on 3 June 2010 with chest pain.
She underwent an appendectomy two days ago
Initially, on 03 May 2017, Mr David presented with a complaint of stomach ache.
Present perfect : started in the past, still relevant now.
(e.g., since/for/over the past week/ recently....)
..... has been/ has improved
Ms Williams's mobility has improved over the past week .
He has reported persistent lower back pain for the last two weeks.
She has experienced shortness of breath during mild exertion.
She has not attended any follow-up appointment since the surgery.
I have completed the X-ray and forwarded the report to the GP.
The radiographer has completed the X-ray and forwarded the report to the GP.
The physiotherapist has recommended daily exercise to improve mobility.
The dietician has explained lifestyle modifications to manage hypertension.
