I am writing to refer Mr Shepherd, who requires your assistance to apply for financial aid and a 4-week-leave from work. He is recovering from a heart bypass surgery.

Mr Shepherd was admitted in our hospital on 20/12/2017, and will be discharged tomorrow. Although he has made an encouraging recovery, he has been advised to get sufficient rest and resume work after four weeks.

Mr Shepherd works as a Business Development Manager at a bank, and is worried about maintaining his employment and taking time off work for a month. In addition, he is concerned about experiencing financial strains owing to a potential reduction in income until he resumes work. 

Given the above, it would be appreciated if you could inform Mr Shepherd's employer of his situations and arrange a 4-week-employment leave for him. Please assist him in applying for financial aid to minimize his stress during the recovery period. Noteworthy, he was educated on smoking cessation; however, kindly refer him to Quit Line for smoking cessation and alcoholic consumption.

Enclosed herewith is the supportive documents regarding Mr Shepherd's medical condition.
