                     Carpenter. Limited contacts

Date of admission: 28/01/2019

Reason for admission: 

                                           Fall onto R hip -suspected fracture.                                                                                             Possible concussion as pt was unsure if                                                                                     head injury/contact occurred at time. Pt                                                                                     intoxicated upon arrival from retirement                                                                                     home. 

Allergies:                           Nil

Social History:                  Resides at Newport Retirement Home

                                          Self-reliant

                                         Heavy alcohol consumption dependency                                                                                   (spirits) 8-10 daily.

                                         Ex-smoker, 35 years (ceased 12 yrs)

                                         Uses own vehicle for short trips, e.g.,                                                                                        groceries, Dr. appointments

Medical History:

                                        Bilateral lower extremity edema cellulites,                                                                                hypertension, Eczema, venous statis                                                                                          hypercholesteromia, morbid obesity,(BMI:                                                                                50.7)

Medication:                    Frusemide (lasix) 40 mg orally twice daily

                                       Topical OTC hydrocortisone

                                       Aspirin 81 mg daily

                                      Rosuvastatin calcium (crestor) 20 mg once                                                                                daily

Medical Progress:

                                     Alert and oriented to person, places, and time

                                    Ambulates independently with walker

                                    X-ray: nil fracture

                                    Some superficial bruising  (reddish/deep                                                                                   purple in colour) - R hip, Skin: intact

                                  Incontinent of bladder and bowel at times

                                 Appetite is fair, no issue noted -chewing or                                                                              swallowing

                                Very intent on healthy eating high sodium foods

                                Wishes to reduce alcohol consumption

                                Senses within normal limits (wears reading                                                                                 glasses)

                                Has personal emergency response system (tested                                                                      during visit with response time less than 2 min. Pt                                                                    encouraged (agrees) to wear all times)

Discharge Plan : 

                                Referral for social worker - assistance with                                                                               arranging

                               Homecare -2x weeks 3hrs (household                                                  c                                     chores/bathing esp. of lower limbs

                              Professional counselling for alcohol dependency

                             Meals on wheel (meal delivery service)

WRITING TASK

Using the information in the case notes, write a referral letter to the social worker outlining all necessary information related to the patient's ongoing care and support. Address: Ms Megan Simms, social worker, 99 Beacon Road, Newport. 

In your letter:

* Do not use note forms

* Use correct letter format

The body of the letter should be approximately 180-200 words.
