Read the case notes and complete the writing task which follows.

NOTES

Patient details:      Mark A Nelson

Age: 60 yo

Social/family background:       Retired, work as a mattress salesman

                                                    Married, wife (55) the primary caregiver

                                                    Two children, both visit them once a month

Medical History:                    

11/02/18                                   Admitted to the orthopedic department of the Shepton                                                               Treatment Centre with worsening L groin pain. 

                                                Exam. revels an antalgic gait with a negative Trendelenburg                                                    test.

                                                Thomas' test. reveled a 10" fixed flexion deformity with                                                          further flexion to just over 

                                                Combined internal rotation and hip flexion consistently                                                            reproduced groin pain. 

Discharge:                              17/12 /2018

Diagnosis:                              Osteoarthritis

Management:                         Total hip arthroplasty

Medications:                            Analgesic use included non-steroidal anti-inflamatory drugs  -                                                 naproxen

Nursing management 

and progress:                      An Oxford hip score of 17/48 was recorded immediately                                                         post- operative.

                                                  Staples removed around 2 wks after surgery.

                                                  Routine face-to-face outpatient review at 6 wks 

                                                  Post-operation revealed a pain-free L hip, a negative                                                               Trendelenburg test and a satisfactory X-ray film. 

Discharge plan:                      Pt's wife educated on pain medication and incision care.

                                                 Instructed to wear the support stockings given by the hospital                                                   for 24 hrs-a-day for 3/4 wks .

                                                Home visits arranged with local community nursing sevice on                                                 wife's request.

Home visit: 

05/03/2018::                           Complained of severe hip pain. 

                                              Swelling, tenderness, and redness in the calf.

                                              Fever of 100.4 F, HR - 112, RR-28, BP - 180 - 90

Assessment:                        ? DVT

Plan:                                    Refer to orthopedic surgeon for urgent assessment and                                                             management


WRITING TASK

Using the information in the case notes, write a referral letter for Mr Nelson to orthopedic surgeon, Shepton Mallet NHS Treatment Centre , VIC 3188 

In your letter: 

* Expand the relevant case notes into complete sentences
