Dear Ms. Simms

Re: Mr Ronald Scott

Age: 72 years

I am writing to refer Mr Scott, who requires your assistance for arranging  two weeks ongoing care and appropriate management. He was admitted in our hospital yesterday with a suspected fracture of the right hip and possible concussion due to a fall. 

Mr Scott was monitored over night and was given appropriate treatment.  The X-ray report ruled out the possibility of fracture. Currently, he can mobilise with the help of a walker; however, he has incontinence of bladder and bowel.

Mr Scott lives alone. Although he has a heavy alcohol consumption dependency, he wishes to reduce it.  He has bilateral lower extremity edema cellulites, hypertension and morbid obesity (BMI 50.7).

Given the above, it would be appreciated if you could arrange a three-hour-home care for two weeks in order to support him for bathing and domestic chores. Besides, please arrange professional counselling for alcohol dependency and nutrition education.  Noteworthy, he has a personal emergency response system to wear all times. 

Mr Scott is being discharged today.
