Presenting complaint: : Anaemia (Diagnosed by the referring GP)
History of presenting complaint:
Irritable, withdrawn, moody lethargy, mild depression
Weight loss- 5 kilos
Past Medical History: Asthma, for 5 years
Typhoid 2009
Chicken pox 2001
Blepharites 2015
Anthropomorphic Data: Height: 181 cm
Weight: 67 kg
BMI: 20.5
Biochemical Data: FBE
Hb: 118g/L (Ref: range: 130-180)
Serum iron : 12 mol/L (Ref: range: 10- 30)
Ferrin: 30g/L (ref: range: 30 -300)
B12: 200 mol/L (ref: range: 120- 680)
Serum folate: 36 mol/ L (ref: range: 7 - 45 mol/L)
Medication: B12 (1000g) IM injection, every 3-6 months
Ventolin inhaler
Social History: Vegetarian for 6 years; eats egg.
University student, lives independently; Share cooking.
No partner.
Body conscious, work out at gym 2 hrs each day6
Non-smoker; no drugs use.
Social drinker (weekends: 3 drinks at night)
Amino acid supplements
Dietary Information:
Breakfast : Cereal with low-fat-milk
Toast (white bread) with jam, honey, or peanut butter
Orange juice
Coffee
Morning tea: Whole meal salad sandwich
Cola or sports drinks
Lunch: Pasta with veg sauce Cake
Tea or coffee
Mid-afternoon: High protein cereal bar or high protein milk shake
Dinner: TV dinners/frozen dinners - soups, pasta with vegetarian sauce, rice,
potatoes, ice creams &/or tinned fruits
Supper : Dry biscuits with cheese or chocolate bars, coffee
Management
Advice given: Increase veg source of iron -e.g., legumes, green leafy vegetables, dry apricots, figs, nuts combine with vit C containing foods (to increase absorption)
Reduce exercise (hour/intensity) to increase iron absorption
Immediate Plan: Draw up dietary menu plan
recommended fresh food especially fruits and veg
iron supplements
Patient willing to discontinue IM B12 - discuss with the GP
Patient believes that his diet is good - reluctant to change exercise habits.
WRITING TASK
