Read the case notes below and complete the writing task which follows.

NOTES:
Assume that today's date is 25 October 2018
Darren Gilroy was referred to you by his GP for dietary advice. 

PATIENT DETAILS:

Presenting complaint:             : Experiencing fluctuating weight problems (between 173 and 191 pounds over the past 3 years). His GP believes that Mr Gilroy has increased health risks because of a "probable metabolic syndrome" 

History of presenting complaint: 
                                              
                                                  Irritable, withdrawn, moody lethargy, mild depression
                                                  Weight loss- 5 kilos

Past Medical History:
23 Feb 2018                             Hypertension; taking Atenolol and aspirin for two years  
                                                 Dyslipidemia, pre-diabetic,  
         
Anthropomorphic Data:      Height: 181 cm
                                                Weight: 67 kg
                                                BMI: 24.5 

Biochemical Data:  FBE
                                               Hb:123g/L (Ref: range: 130-180)
                                               Total cholesterol : 249mg/dL
                                              Triglyceride level : 297 mg/L
                                           
Social History:                    Aerobic exercise five times/wk 35 min. session
                                             Has difficulty eating less fatty food to lower his cholesterol

Dietary Information: 
                     Breakfast :      Cereal with low-fat-milk
                                             Toast (white bread) with jam, honey, or peanut butter
                                             Orange juice
                                             Coffee
                    Morning tea:  Whole meal salad sandwich
                                            Cola or sports drinks

                           Lunch:    Pasta with veg sauce Cake
                                           Tea or coffee
                          Mid-afternoon:  High protein cereal bar or high protein milk shake 
                          Dinner: TV dinners/frozen dinners - soups, pasta with vegetarian sauce, rice,
                                           potatoes, ice creams &/or tinned fruits
                          Supper :  Dry biscuits with cheese or chocolate bars, coffee

Management
Advice given:                 Was assessed to be at the "action" stage of the readiness-to-change behaviour model for physical activity  and at the "preparation" stage for managing weight and choosing low-fat-foods.

Anthropometric data:     Weight : 165 lbs (BMI: 25.8) 

Lab reports:                 FBC: 109 (down 14 mg/L )
                                       Cholesterol : 226mg/L (down by 23 mg/L)
                                      Triglyceride level: 185  (down by 143 mg/L) 
                                      Patient wants to lose an additional 10 pounds for a good weight of                                                        155pounds 
Plan:                            Continue Atorvastatin ; Review after 2 months

WRITING TASK

Using the information in the case notes, write a letter  to the referring  Dr Entwhistle, outlining your assessment of Mr Gilroy, his progress-to-date, and your management plan. Address the letter to Dr Bert Entwhistle,  Poly Clinic Hospital,  345  street, Newport.

In your letter: 

* Expand the relevant case notes into complete sentences

* Do not use note forms

* Use correct letter format

The body of the letter should be approximately 180-200 words.
