Read the case notes and complete the writing task which follows:

Assume that today's date is 20 July 2020

Mr. Matthews was referred to you by his PCP for weight loss management.

PATIENT HISTORY

Name: Gerald Matthews 

DOB: 01/06/1970

Social background: Librarian of the Chicago University at Columbia.

Wife: Died six months ago, no children. Works a six -day-shift.

Medical History: Hypertension, Hx. 4 years

                              Obesity: 3 yrs

Recent test results:    Cholesterol:  5.90 mmo/L, (incr from 4.8 - target less than 4.5)

                                   Triglycerides: 4.10 mm/L (Target less than 2.0)

Gained weight: 6 kg in the last 3 months.

Diagnosed diabetes 10 months ago.

Current weight: 140 kg

Height:   176 cm

BMI: 42.3 (ideal  20-25)

Life style: Sedentary, walking difficult (- hypertension)      

Referred by Dr. Wolfe, for management  of 0 chol, 0 triglycerides & wt loss.

Breakfast: Dry chappathis (3),  small banana, cow milk,  Tea (no sugar).

Snacks: Samosa, tea (as above) 

Lunch: Rice (2-3- cups), Butter, fish fried, cola.

Snacks: Beef rolls/samosa.  

Dinner: Rice 3-4 cups, Red meat curry, coconut milk, tea.

After dinner:  Chips, nuts, sweet biscuits, Lays.

Summary of intake:  Multiple attempts to weight loss

                                Currently eating regular meals, making healthy choices. (low fat dishes,                                  fish, beef,  no sugar, snacks in the evening, tea as usual.

Suggested meal plan:   Regular meals: Include healthy snacks between meals, avoid                                                    snacking on unhealthy foods, 0 fat intake ( low fat dairy foods,                                                smaller portions of meals, light coconut milk in cooking.)

                                     Controlled portion sizes - e.g., half meat serves, o intakes of breads,                                       rice meal - plan provided)

Writing Task

Using the information given in the case notes, Write a letter to the referring doctor describing the current situation and changes made by the patient. Address the letter to Dr . Richard Woolfe,  27 Green City, No.2 St, Adeline.

In your letter:

        * Expand the relevant notes into complete sentences

        * Do not use note forms

        * Use letter format

Your letter should be approximately 180- 200 words
