Re: Mr Brad Jenkins

DOB: 05 June 1972

I am writing to inform you about Mr Jenkins, who is scheduled for an X-ray of the ribcage tomorrow. He is a a psychiatric patient with a history of aggressive behaviour. 

Mr Jenkins's X-ray was scheduled for the procedure today at 2.00 pm, and he presented at the right time attended by a psychiatric nurse. However, he was unhappy to be X-rayed.

Considering the condition of Mr Jenkins, the Chief Radiographer, Mrs Hilda Vickers, instructed that he should not be left alone with a single staff member, and both a radiographer and a non-radiation worker be present in the X-ray room for the procedure. If a non-radiation worker is not available, a staff member from another ward can be called upon to assist the procedure.

However, after five minutes,  the orderly in attendance was called away  to assist with an urgent image intensifier procedure. When we informed Mr Jenkins about the delay another one hour, he became angry and threw away a chair in the waiting room. Earlier, he had caused a violent incident which was the cause of the injury in the ribcage.

In view of the above, please ensure care taken when X-raying   Mr Jenkins tomorrow.
