Setting: Community Health Centre

NURSE: You see the parent of a 2-year-old who has come for their routine vaccination appointment. You are providing some information beforehand. The child is sitting nearby.

TASK

* Explore the reason for the visit. Confirm the child's age and child's vaccination history (e.g., previous vaccines, any missed doses). Find out any specific questions or concerns the patient may have.

* Describe the purpose of the of the vaccines ( e.g., protects against serious diseases such as measles, mumps, rubella, meningitis, and polio). Explain the need for vaccination (build immunity before exposure to diseases, herd immunity).

*  Explain the process (quick injections, slight pinch).  Suggest comforting techniques(e.g., holding the child, distraction). Find out child's health issues ( any known allergies, currently unwell, etc.).

* Outline potential common side-effects (mild fever, soreness at injection site, irritability). Advise managing these side-effects at home (e.g., paracetamol if needed, cool compress, lots of fluids)

* Reassure about safety (e.g., (vaccines tested thoroughly and monitored: serious side-effects are extremely rare) Find out any concerns or questions.

* Give details about post-vaccination care. (e.g., injection site clean, avoid rubbing vigorously, observe for unusual symptoms).

* Explain when to seek further medical advice (e.g., high fever, difficulty in breathing, rash).

* Establish parent's consent for the child's vaccination.
