You see a mother whose 9-year-old son has just had a laproscopic surgery (appendectomy) . The son is ready for discharge, and you are to discuss the care plan with the mother and address her concerns.
TASK
* Confirm your reason for the your visit
* Reassure the patient's readiness for discharge (son's recovery is satisfactory, stable condition, able to tolerate fluids. Find out if the mother has any immediate concerns. (eg. pain, diet, resuming normal activities, etc..)
* Outline common temporary after-effects of the surgery (mild abdominal discomfort, gas pain, fatigue, slight bruising around the incision sites) . give details of the recommended pain relief for a child (prescribed medication , appropriate dosage, over-the counter options like children's paracetamol, avoid aspirin).
* Advise on diet and activity expectations (start with light bland foods, gradually return to normal diet as tolerated). Give guidance on physical activity (avoiding strenuous play, heavy lifting or rough activities for a specific period, emphasising a gradual return to school and play).
* Recommend wound care to prevent infection (keep the small incision sites clean, and dry, how to bathe or shower etc)
* stress the need for medical help if if the child develops severe symptoms ( eg. high fever, severe, vomiting, abdominal pain , or signs of infections at the incision sites). Confirm follow-up appointment in two weeks, in-patient department)
