Dear Mrs Carl
Re: Mr Ross Sergio
BOB: 13/03/1969
Thank you for accepting Mr Sergio, a 57-year-old airport handling staff, who requires further monitoring and patient-care. He is recovering after a left total hip arthroplasty and is being transferred to your care.
On 04/02/2022, Mr Sergio underwent the surgery, which was largely uneventful. The implant used was acetabular cetabular shell 60 mm with a 6.5 mm x 30 mm cancellous screw 0-degree polyethylene with 127- degree neck angle hip stem.
During hospitalisation, Mr Sergio was managed with Amlodipine (10 mg) in the morning, OxyContin (80 mg) Q12H, Accetaminophen (650) Q6H and aspirin (325 mg) twice-a-day.
As per the transfer plan, please monitor Mr Sergio's progress. I would be appreciated if you could educate him regarding the hip precautions. Please note, he requires supervision and arrange abduction pillow if required. Noteworthy, his suture is to be removed in two weeks.
In view of the above, Mr Sergio is being transferred to your care.
Should you have any queries, do not hesitate to contact me.
Yours sincerely
Doctor
