Dear Dr Nunetan
Re: Mr George Bush
DOB: 01/03/1958
Thank you for accepting Mr Bush, a 63-year-old truck driver, who requires partial glossectomy with hemi-mandibulectomy and radical neck surgery. He is a case of stage IV squamous cell carcinoma and is being transferred to your facility today.
Initially, on 05/01/2022, Mr Bush was suspected to have a tumor at the base of his tongue as revealed by his FNAC, CT and MRI reports. Therefore, he was admitted for observation and further investigations. The next day, his test reports showed poorly differentiated squamous cell carcinoma and metastatic carcinoma, which required further investigations including the staging of the cancer.
Today, the pathology results of Mr Bush confirmed a stage IV squamous cell carcinoma of TNM staging T2N2bMO. However, the CT of chest reports show no metastatic involvement of the chest. The pathology reports are Hb: 143, Platelets:224, WBC:7.2, and creatinine: 83 mmol/L. His liver function, TSH and albumin are normal.
As per the transfer plan, Mr Bush requires the surgery and patient motivation.
In view of the above Mr Bush is being transferred to you for further assessment and management.
Should you have any queries, do not hesitate to contact me.
Yours sincerely
Doctor
