Thank you for seeing Amina, a refugee from Somalia, who shows signs and symptoms suggestive of meningococcal meningitis and requires an urgent admission for the definitive diagnosis and further management of her condition.
Today, Amina reported for a review with the complaints of vomiting and severe headache. She was lethargic and listless. On examination, she has high grade fever (40.2 C) and signs of maculo-papular rash over the legs as well as elevated neck stiffness. Her blood test reports show an increased white blood cell count with a left shit, but her urine function reports are normal. Therefore, she is suspected to have meningococcal meningitis and was administered intravenous penicillin soon.
Initially, on 09/10/1912, Amina presented with the complaints of fever, runny nose, mild cough, loss of appetite. She was febrile with a high temperature 39.4, but with no signs of rash, nor neck stiffness. Her systemic examination results were normal, which led to the assessment of viral infection. She was prescribed paracetamol, three times a day, adequate rest and off from school.
However, on a review on the third day, Amina's temperature and cough were elevated, but the other conditions showed no signs of improvement. Therefore, she was prescribed Brufen 200 mg (as required) along with a full blood count and a urine function reports were ordered.
Today's review confirmed the diagnosis of meningococcal meningitis and she is being referred to you for the specialist's intervention.
