Role play Card.

Setting: Health Clinic

Doctor: A 62-year-old patient presented two months ago with herpes zoster  (shingles) . He/she is now re-presented with the complaints of post-herpetic neuralgia (the nerve that often lingers after a shingle rash goes away).


Task: 

        * Explain to the patient the cause of the pain

        * Recommend common analgesia as the first line treatment and explain the after-effects of strong pain medications (opioids)

        * Give Capsaicin cream for topical application 

        * Explain the side-effects and steps need to be followed while applying the Capsaicin cream (e.g., a slight stinging or burning sensation o the skin, wear gloves        while applying the cream to the affected area)

       * Prescribe anti-convulsant drugs and explain the effects of it  (Gabapentin or pregabalin may be useful, if necessary)

      * Explain that non-pharmacological may help (e.g., relaxation - meditation, adhesive dressing and cognitive    behavioral therapy) 



Interlocuter: Good morning, could you please tell me your full name?

Candidate: Good morning, and my name is Dr Zusaki Sahid.

 Inter: Can I see your ID, please?

Dr Sahid: Well, hers's my passport which I used for registration.

Inter: Thank you. Can you please tell me your candidate number, please?

Dr Shahid: Well, my candidate number is: 04 67 86 21.

Inter: Thank you. And you are taking this test as a medical doctors, am I correct?

Inter: Now, we'll embark on the warm up questions; they are not assessed and is a chance for us to get used to each others' voices. Well, let's talk 2 to 3 minutes.

Dr Shahid. Ok.

Inter: Great. Why did you choose OET?

Dr Shahid: Well, I believe that OET is the best language test a medical professional can choose as it deals with real-life scenarios and I personally believe that every medical professional will feel confident and comfortable while speaking or something related to medical field.

Inter: Thank you. How comfortable are you working with other doctors or nurses ?

Dr Shahid: Well, I work well under a variety of conditions and circumstances. I take  pride of my flexibility; I really enjoy working with a big  team. I often find that different viewpoints can help me find solutions to a problem that I might not think of my own. I, also, enjoy self-motivating.

Inter: Are you a team player? Tell me about a time when when you worked well with a team .

Dr Shahid: I would say that I'm a good team player. As I said , I like to make sure that my team knows that I'm there for them and they make sure to support all my colleagues' decisions. Moreover, since most of our approaches are multi-disciplinary, we put our heads together before and after any procedures so that we can turn any difficult situations into an opportunity to grow.

Inter: Great. What are you passionate about?

Dr Shahid: Well, I'm passionate about rendering the absolutely best care to all my patients. I take pride in my work and I'm passionate about making a difference in people's lives. 

Inter: Thank you. Talk about a time when you had not communicated well and how did you correct the situation.

Dr Shahid: Well, this happens quite often in our profession  when there's a language barrier. I learned that it is always better to ask for an in-person interpreter or utilizing a computer translation software. This ensures no miscommunication of information. Unfortunately hand gestures and speaking louder always does not work always.


Speaking:

Dr: Good evening. I'm Dr Shahid, one of the doctors in this Health Clinic. May I know, how can I help you?

Pt. Good evening doctor. I had consulted in this clinic for two months for the treatment of shingles that appeared on on my neck area and thankfully I recovered completely. But unfortunately, I'm experiencing some sort of burning tingling and severe pain over the area which was affected with shingles. 

Dr:  If you don't mind, may I assess the affected area?

Pt. Yes, sure.

Dr: For my clarification, shall I ask you a few questions?

Pt: Of course, Dr.

Dr: Have your symptoms been continuous or occasional?

Pt: Well, I can say that the symptoms are intermittent. 

Dr: Alright. May I ask you, how severe are your symptoms? For making it easier, how would you rate your pain on a pain scale of 1 to 10, where 0 means no pain and 10 means severe. 

Pt: Well, It can be considered approximately as 5 and above.

Dr: I'm Really sorry to hear that. Any way, don't panic. You are here to manage it effectively. 

Pt: Thank you Dr, but I'm Completely unaware of the cause of these symptoms. 

Dr: Well, your anxiety is quite normal. However, don't worry. I can explain it to you. Your symptoms are those of post-herpetic neuralgia, as we doctors call it. It's the most common complication of shingles. The risk of this condition increases with age, primarily affecting older people, over 60 years. The condition affects nerve fibers and skin causing burning pain that last long after the rash and blisters of shingles disappear.

Pt: A complication of shingles? Well, doctor, how long will it last?

Dr: Unfortunately, there's no cure, but treatment can ease the symptoms. Still, there's hope that in most cases, people with post-herpetic neuralgia improve over time. 

Pt: It's very hard for me to accept, yet I would like to know about the treatment options available for my condition. 

Dr: Fine. I'M really glad to explain it to you, but before thatI would like to know, how long long you were experiencing these symptoms? 

Pt: It has been for the past two weeks, doctor.

Dr: OK. So, as the first line treatment, I can give some painkillers, such as tramadol. 

Pt: But, doctor, I wonder if my pain subsides with these medications. 

Dr: O! Yes, I can also provide a capsaicin cream along with the painkillers. The cream is made from the seeds of hot chilly pepper which might reduce the pain from post-herpetic , but this cream may produce some side-effects, such as irritation and burning sensation on the skin, but it usually disappears over time. 

Pt: OK, Doctor. 

Dr:  An important thing to be noted, you should always wash your hands, and wear gloves while you are applying this cream on the affected area. 

Pt: All right, doctor, but if you don't mind, some high dose pain killers?

Dr: Well, high doses of pain killers may have severe side-effects: addiction and even death. Well, if you are experiencing severe pain, I can give you some anti-seizure medications including gabapentin and pregabalin which could lessen the pain. These medications stabilize the abnormal electrical activity in your brain caused by the injured nerves; mind you, it has some side-effects, including drowsiness, nausea, unsteadiness and swelling in the feet. 

Pt: Ok, doctor, but I'm afraid of taking medications for a long time. 

Dr: Well, I acknowledge your situations, still you are supposed to follow medications until your symptoms subsided. 

Pt: Yes, doctor, I understand the importance of taking medications.

If you don't mind, shall I ask you that is there any alternative treatment options available for this?

Dr: Yes, indeed. You can go with some non-pharmacological management, such as protecting the skin from external stimulations by covering the affected area with a clear adhesive dressing. Secondly, stress, anxiety and distress exacerbates pain. Therefore, relaxation therapy and cognitive therapy will relieve the pain to a greater level. In addition, acupuncture is also considered an effective pain management strategy foe a minority of people. Do you follow me?

Pt: Yes, doctor. Thank you.

Dr: Any how, as I told you already, there's some probability of recovering over time. Please come for a review after 2 weeks.
